General Practice Nurse
Job Type: Full-Time
Location: Altona North
TLC Healthcare is currently seeking a motivated and compassionate General Practice Nurse to join our growing general practice team in Altona North on a full-time basis.
Working closely with GPs, the Practice Manager, and a multidisciplinary team, you will play a key role in delivering high-quality, patient-centred care and supporting improved health outcomes for our community.
Key Responsibilities
- Provide safe, evidence-based nursing care in a general practice setting
- Conduct patient assessments, care planning, and follow-up
- Support health promotion, chronic disease management, and preventive care
- Assist with recalls, reminders, and care coordination
- Maintain accurate clinical documentation and patient records
- Work collaboratively with GPs and allied health professionals
- Contribute to quality improvement and accreditation activities
About You
- Previous experience working in a Medical Practice
- Strong understanding of infection control, clinical standards, and patient care
- Excellent communication and interpersonal skills
- Ability to work both autonomously and as part of a team
- High attention to detail and commitment to quality care
Qualifications & Requirements
Relevant nursing qualification
- Current AHPRA registration
- Nurse Immuniser qualification (or willingness to complete training)
- Up-to-date COVID-19 vaccinations (minimum 3 doses)
- 2026 Influenza vaccination
- NDIS Worker Screening Check (includes police check)
- Aged care experience is highly regarded
- Full working rights in Australia
