The Care Manager will thrive being on the floor to provide hands-on quality nursing care to our residents

 

Care Manager (Division 1 / Grade 5) – Armstrong Creek, Geelong

Location: Warralily Gardens, Armstrong Creek

Monday to Friday | 07:00 – 15:15 (with operational flexibility)

Your Role as a Care Manager (Division 1 / Grade 5), you’ll be central to clinical care and team leadership, providing high-quality, person-centred support to residents and their loved ones. You’ll thrive as a hands-on leader in a collaborative and respectful culture.

Key Responsibilities

  • Deliver comprehensive nursing care tailored to individual needs
  • Oversee medication management and wound care
  • Collaborate on care planning and clinical recommendations
  • Drive continuous clinical improvement
  • Build strong relationships with residents, families, and visitors
  • Ensure timely assessments, care plans, and documentation
  • Mentor staff in clinical education and competency
  • Foster an inclusive, values-driven team environment

What We’re Looking For

·         Registered Nurse – Division 1 with AHPRA Registration

·         Strong clinical experience and leadership skills

·         Passion for aged care and person-centred support

·         Excellent communication and professional presence

·         Commitment to quality improvement and team development

 

Things that matter to us 

  • Two years’ experience in a clinical leadership role 
  • Understanding of the Aged Care Quality Standards   
  • Relevant Nursing Qualifications (Division 1) 
  • Current registration with AHPRA 
  • Up-to-date COVID-19 vaccinations (3 doses)    
  • 2025 Influenza vaccination
  • NDIS Check (includes Police Clearance)    
  • Eligibility to work in Australia
